Graduation Weekend Space Reservation form

Typically this is the third weekend of May. Requests for the following year will be accepted beginning June 1 and is on a first come, first served basis. ONE 6-HOUR SLOT PER FAMILY.

Steps

1. Check the church calendar for availability.
2. Complete and submit this form. Make sure you indicate your first and second choices in the event there are multiple requests for the same slot.
3. Your request will be reviewed by the event coordinator and submitted to the Deacon board for review.
4. The event coordinator will notify you of the board's decision.

Reservation and Use Policy

This form must be completed, signed and returned to the event coordinator between June 1 and September 1. After September 1 there is no guarantee that any tables, chairs or other equipment will be available for use for graduation weekend.

Refrigerator space is subject to availability. Check with the other parties to see if they have extra refrigerator space they can share.

I am responsible to set up the tables and chairs as per the location descriptions below. Set up for the morning time slot may begin before 8 AM; clean up for afternoon time slots may take place after 8 PM; and set up for Sunday afternoon may begin after worship services on Sunday.

Tables and chairs set up for Friday may stay in place for Saturday events. Those scheduled Saturday night must return all chairs, tables, and equipment back to their proper places that evening.

The user/group agrees to indemnify and hold harmless Community Bible Church and its representatives from all damages, judgments, expenses, attorney's fees and compensation arising out of personal injury, death or property damage sustained in whole or in part by any or all persons whatsoever as a result of or arising out of, any act or omission of user/group, its agents or employees, or caused by or resulting from any activity incident to the program being conducted by the user/group.

Certificate of Liability Insurance: If your organization carries liability insurance, please request your insurance carrier to add Community Bible Church as an additional insured and provide us with a certificate of insurance listing Community Bible Church as the certificate holder.

• CBC operates on a "leave it as you found it" basis. Set up and tear down is responsibility of the user.
• Our facility cannot be used in business or for a “for profit” event.
• Groups are restricted to only those areas of the facility that the user has reserved.
• Food and beverages should remain within approved areas.
• Church equipment, such as tables and chairs, must be returned to original location.
• Clean-up is the responsibility of the user.
• Neither alcohol nor smoking is permitted on CBC premises

I have read and understand the CBC Facility/Equipment Use Agreement and agree to all the stipulations therein. I understand that this application must be approved, and I must pay half of the total cost of any usage fee in order to schedule my event or reserve equipment.

COFFEE LOUNGE: This area has a sink. If kitchen space is needed please work with other families using the other areas. The space includes room 400, as well as 8 round tables, 64 chairs and 2 rectangle tables.

FELLOWSHIP HALL: This includes the fellowship hall kitchen, room 109, as well as 8 round tables, 64 chairs and 2 rectangular tables.

FAMILY LIFE CENTER: This includes the gym, the FLC kitchen, room 200, 10 round tables, 80 chairs, and 2 rectangular tables.
