Graduation Weekend Equipment Reservation Form

Use this form if you wish to borrow equipment for graduation weekend.

CBC welcomes members and regular attendees to take advantage of the equipment to meet their needs. Non-members are encouraged to check local rental agencies prior to filling out request forms for use of CBC equipment; and are required to have a CBC member as a sponsor.

Please keep in mind, the church may not in good conscience materially cooperate in activities or beliefs that are contrary to its faith. Allowing its facilities and/or equipment to be used for purposes that contradict the church's beliefs would be material cooperation with that activity and would be a serious violation of Community Bible Church's faith and religious practice. (2 Corinthians 6:14, 1 Thess. 5:22)

PLEASE NOTE: The following items are not available to be loaned out to members or non-members: snow blower and lawn equipment.

Steps for Reserving Equipment

1. Complete and submit this form.
2. Your request will be reviewed by the event coordinator and submitted to the Deacon board for review.
3. The event coordinator will notify you of the board's decision.

Reservation and Use Policy

I understand: This form must be completed, signed and returned to the event coordinator between June 1 and September 1. After September 1 there is no guarantee that any tables, chairs or other equipment will be available for use for graduation weekend.

The user/group agrees to indemnify and hold harmless Community Bible Church and its representatives from all damages, judgments, expenses, attorney's fees and compensation arising out of personal injury, death or property damage sustained in whole or in part by any or all persons whatsoever as a result of or arising out of, any act or omission of user/group, its agents or employees, or caused by or resulting from any activity incident to the program being conducted by the user/group.

I have read and understand the Community Bible Church Facility/Equipment Use Agreement and agree to all the stipulations therein. I understand that this application must be approved.

Members of CBC use the equipment without cost; however, should damage occur to the building or equipment therein, replacement or repair will be negotiated. Do not drag the tables over concrete/rough surfaces as it damages the tables.

CBC operates on a "leave it as you found it" basis. Equipment should be returned to the place it was borrowed from in the same condition (clean).
